Default putting an exhaust brake on a stock 2004.5 48re truck

I did a thread search and couldn't find anything on this but maybe I didn't know where to look. Does anybody have any experience with this and is it a good idea? I have a combined gross of around 17k and the stock setup does well on the interstates with 6% grades but the back roads with 10%+ leaves me wanting a little more hold back. Is there something that needs to be done to the 48re to make this doable and what would it cost?
