I purchased it through Summit Racing along with the ramair induction kit. It was about $705 including shipping, the ramair was another $309 including shipping, then it cost about $370 to have it dealer painted and installed. I had found some cheaper wanted to tick with a company that has a good reputation (returns etc.)
It is made by Keystone Restyling it's one of their GoodHoods.

according to the book, its supposed to add 15 or more hp, and increase gas mileage, I haven't driven enough to check it all out yet.
