Could it be the notorious broken exhaust manifold stud issue with the 4.7's? Does the clicking ever go away when it warms up?

No, acutallly you don't hear until after it beens running for about 5-10 mins. I took it by a shop and they said it sounded like a fan clutch bearings are going out. But the fan is spins off the water pump and I took one of those stethlo scopes and listen to it and it wasnt anything unsualy but truck has two cooling fans and the other I have not listen to yet but could that be it.? Also are they suppose to have two fans?

How could I tell if that is what the problem is?

Look for pieces missing out of the shroud/ or the fan.
