Default P0068 after JTEC to NGC Swap on a '04 Dakota

Hey all, I posted awhile back after I wasn't able to get my '04 Dak to start after swapping the engine from a '02 Durango. Well we were able to get it started after figuring out the issue was in the tone rings. After taking the engines back apart and swapping out the tone rings however it started right up and runs just fine for maybe 2-3 minutes.. after that the engine throws the code P0068 for the MAP sensor and starts surging, Idling at about 1400 and reving up to 2k-3k. We've already tried replacing the MAP sensor and the issue still persists.

Any help would be much appreciated!

Map code. You may have a vacuum leak somewhere.
