Default Rear heater core hose

I have a 98 Grand Caravan and recently found out that the rear heater core hoses have eroded away and ruptured. As a result
the entire contents of my radiator leaked out and left me with an overheating engine. I would like to be able to replace these myself and
would like to find out where the hoses connect to inside the engine. I followed the hoses under the car and they end up going into the engine
but I can't figure out where they wind up? Any help?

Thanks

I had the same problem with my 96 in -40 weather. The garage said the only repair was to replace the coolant hose assembly which coincidently was in stock at the dealership.

I replaced the lines on my 1996 Voyager in 2003 and now have to do the same on my 2002 Town & Country. At least now the lines are aluminum in place of steel. Still a bit** to replace.

To replace that hose assembly you have to remove the wiper transmission assembly, and on the 96 - 00 minivans the upper intake plenum has to be removed as well. Oh its a pain alright...the 01 to present minivans only the wiper assembly has to come off.

Going through this same grief right now on a 2001 Grand Caravan. I've tried every which way to get the old one out. Any wisdom would be greatly appreciated (aside from taking it to the dealership).
