Have a back pressure test done on your CAT. You should run under 3psi. A muffler shop can do this for you and is usually free of charge.

Transmission filter/fluid might need changed. Filter/fluid should have been changed with the trans rebuild in JAN08 I would like to think that they didnt use the same filter and fluid that was a causal factor in the transmission rebuild in the first place. You may be on to something it has been 25K since rebuild so that is a good possibility.
Does it run hotter than usual?
NO, was running slightly hotter when water pump was failing and leaking DexCool but back to normal temps with water pump R/R.
Also, Never trust the overhead unit! Always check your actual numbers at the pump.
I use the O/H as a reference but still back up with actual and the mileage I am talking about is the actual mileage.

Update.
Took it to Goodyear, he looked at me like I had three heads when I asked for back pressure test, and insisted that if CAT or O2 sensors were bad they would trip a code. I asked if they could be blocked enough to give bad fuel mileage but not enough build up to trip a code to the MIL, again the three head answer. Next place is a muffler shop, I know Hydra, if I had just taken it to a muffler shop I would be done now right?
Just on a lark I pulled the #1 plug for inspection, I picked #1 for ease of removal and since it is the beginning of the firing order. Compared it to the Haynes plug wear chart in the back insert of their manual. They look normal for heat range and temperature. I am computer illiterate with the photo attachments but I can send to members on request if they need clarification.

Change the pulgs. Try Champion Truck Plugs or go to the OEM Champion again. Remember to gap em.

Still a bit early to tell but filled up the tanks after shocks were installed and so far I am already picking up about 20-25 miles per tank. I am back into the 300+ club again, I will monitor with actual during this tank but last tank was 258 miles and fill up was 17.5 Gallons, so just barely over 15 MPG.
FYI As for the photos, I agree with Hydra, Indy you are wrong. I wont ask where you got the photos, I am sure I do not want to know.
