1) give us all the information that we need, which transmission do you have? and which engine do you have? and which driveline do you have? these will all help us determine what electronics and wiring we are looking at.

I need to know if you have a 4.7 engine which would have a 45RFE transmission, 5.2 with a 42/44RE or 5.9 with a 46RE transmission?

When the 46RE shifts from second to third, the front clutch applies..The front clutch is also applied in reverse, So if reverse is 100% with no fault, then the other issues are low governor pressure, high throttle pressure or the 2-3 shift valve in the Valvebody is stuck or the low possibility of the 2-3 servo not releasing.........these can be broke down one by one, but if reverse has any kind of a slip then its time to remove and repair ...

At this point what i would advise is to unplug the 8-pin connector next to the rear cooling line......with harness disconnected you should have low gear in manual low, second gear in manual second and third in Drive position.....the reason for this test is to rule out some crazy electrical concern....if it does not work through the gears then it is hydraulic or mechanical....
