Hey,
I have the same truck - same issue. I don't think it's the transmission so breath a sigh of relief. I am thinking it may be a U joint issue and I will be swapping mine out this weekend. I will let you know if this does it.

Replace the torque converter. This was causing your rumble sound, the torque converter is going into lock up and after its been in lock up for a short time. It starts to rumbler.

Its fixed. Replaced the u joints for the rear driveshaft. No more shudder when starting from a dead stop and no more vibration/ noise when you hit 40-50mph. The u joint going to the diff wasnt too bad but the front one had a bearing cup with all the needles gone. The joint felt good after a physical examination when the driveshaft was on the truck but after I took it off, you could tell the joint was bad.

I am not sure about the uphill but worse when you stood on it as opposed to starting off easy. For $25.00 and a couple hours labor - you will have at least new u joints. I did a step by step on another thread. let me know if you need anything.
