Technically according to the insurance when they gave him 4k they bought it from him, right? So when he bought it back it had changed hands twice... or am i mistaken?

Technically; however, the title never changed hands. I've "bought back" wrecked vehicles before and never had to surrender the title to the insurance company in order to "buy it back". AFIK, the only time a title is going to be branded as a salvage is if the vehicle was not bought back by the owner, hence it officially changes ownership.
