yeah you have to cut a new hole for the setup because the other one will not be in line for the crank shaft. However I just put my spare in the bed.
But yeah you remove something with the tyranny and put something else in. I didn't and it broke and had to pay $400 dollars to fix it.
But really just read the instructions and you'll be fine

Wow lot of responses thanks! They want $450 to do it...so at least I know they are asking fair price! Funny thing is they want the same cost for a suspension lift!?
I wonder is I raise the front 2 inches and the back 1 inch and they do a 3 inch body lift if my 33" tires would just look stupid?!?! I chose 33" cause I didn't want much rubbing and didn't want to have to change my gears...more cost!

I run 6" of lift with 33's. I would much rather look stupid (although I dont really think it does) then have to drive with 35's and 3.92 gears.

I have 2 add-a-leafs in my stock spring packs and it leveled my truck perfectly. But, I also gained 3.5" in the front from the coil springs I got from the local spring shop (I think they are for a diesel truck).
From my understanding, stock v10 and diesel springs will net 3" or so of lift on a half, while the 3/4 ton gassers are more like 2" or so.
