Can you pull the codes from the PCM?

Does the engine seem okay when idling?

My guess is you have a transmission issue but without more info it is going to be tough to be sure.

Probably missing. Should get the codes read, and see what they have to say. Could be any number of reasons for a miss. Plenum bad, fouled plugs, bad fuel pressure, bad plug wires/cap/rotor.... etc.

Is it worse at about 45, when the TCC engages, and in fourth gear?

could be clogged cat for the rattling noise. as for the shaking i dont know. could be front u joints going bad and shaking so bad that it is messing with the wheel speed sensor if you have them.

Flashing cel is severe misfire, the shaking can be from a misfire thats bad enough, but your cel should have stayed on for that.
You really should pull the codes, then come back and post them. Not sure if your in the US or Canada, Mexico, or across the big blue, but, find a place that will plug in your truck and find out what codes are stored.
