Would you believe 10 miles a week..... that's a guess as the odometer doesn't work.... it's on the list.... way down.
Truck starts easily if you fire it up daily and runs smoothly.
Let it sit for a week and it runs rough on start up.... if it is damp it is even rougher.... after a few minutes running it smooths out.
What needs looking at?

you're 02 sensor is getting wet IMO

Or water condensing in the gas tank, or elsewhere in the intake tract.

Gas breaks down over time to....... given how little you drive, how long has the fuel been in the tank?

Moisture may be getting in the distributor cap.

I would check electrical, after gas.

(WET... WATER... and MOISTURE)........ and I'm on a small island surrounded by all 3..... any connection?
Where do I find the 02 sensor and how do I keep out wet?
Water in the tank.... if I top off the tank after every trip will that solve it?... we're talking maybe 6-7 liters each time.
Distributor cap.... can I spray it with that water displacement WD??

1) You can get 02 sensors at pretty much any parts store or rockauto.com
2) NEVER TOP OF YOUR TANK!!! EVER!!! That will just lead to emissions problems among other things. Try adding seafoam to your tank. if you have water in ther, this will take care of it.
3) WD 40 is VERY flammable so I wouldn't suggest it... but then again you are surrounded by water on 3 sides.

Yesterday morning 6am I cranked the engine for 4 seconds then stopped.
Cranked again immediately and it fired right up..... no roughness at all.
Truck was soaking wet with condensation..... damp air.... morning mist on the shore.... temp about 75F.... no wind.
Usually I keep cranking... 7-8 seconds until it starts then it runs rough for 15-20 seconds then smooths out.
What's it telling me? short cranks.
Only the first start of the day is the problem... once you drive it starts easily all day.

Have you checked the fuel pressure....if not check the pressure at the rail
