Surge at idle....still

In a van its real easy to get to the IAC to clean it and the throttle body. But on a truck to properly clean the IAC and throttle body you need to remove the throttle body, because its just about impossible to clean it otherwise.

could it be my timing is off? but wouldn't that affect more than idle?

Timing is controlled by the PCM, not adjustable.
