Question problems with nv3500 5 spd

i just bought a 2000 1500 4x4 5 spd. it wasnt the prettiest truck by any means but i have problems coming from the trans. sounds like constant grinding/vibrations, but it shifts fine and none of the gears have an obvious problem. it has 173xxx miles, is there something i need to replace like throwout bearing maybe? i have no clue at all. and any suggestions on headers, i need new and havent looked at what works good for the rams.

I'd check the t-case, u joints and the like.

Does it seem to actually come from the teams itself or just the drivetrain in general?

Check fluid level, and if it even has the right fluid in it.
