Last time tune-up was done (throttle body cleaned and IAC, plugs, wires, distributor and rotor)?

Need to be more thorough than all that. Disconnect the the battery, pull the throttle body, remove the IAC, and clean everything REALLY well. ESPECIALLY the IAC well. Put it all back together, hook up the battery, turn the ignition to "on", NOT start, count to ten. Start the engine. If everything else is up to snuff, should fix it.

I second cleaning the throttle body and IAC and IAC passage, my truck would randomly stall when going from park to reverse or from drive to reverse when backing out of my parking spot....3 filthy rags and some carb cleaner later the TB and butterfly valves and the undersides as far as my finger would reach was pretty decently cleaned of gunk as well as IAC and IAC passage. I didn't remove mine to do it 100% proper, didn't want to disturb some thick metal fencing wire a previous owner hooked up to the kickdown trans cable to the throttle bracket when the stock plastic end connector apparently broke....hey, if it works...it appears to be stronger now than from the factory :P
if you're gonna do it proper, get another gasket for the throttle body too. Haven't had any issues since, and truck has much better power now too.
