Local upholstery shops should have it, especially ones that specialize in doing headliners. If you do a google search on 'headliner fabric' you will find numerous options. Looks like Hobby Lobby has it, along with Amazon and numerous other places. In my humble opinion, its best to pull out your existing headliner, remove the existing fabric and glue in new fabric, unless you are extremely lucky and can find one in a salvage yard where this has already been done.

I need the part under the fabric too. Do most upholstery shops have it?

No, they won't. In fact, I don't think they are available at all anymore. Best you can do is use yours as a pattern, and cut it out of some suitable material.

LMC uses plastic. I am going to order one and use it. They guarantee it won't sag.

As was stated, the backing board for the headliner is not something an upholstery shop typically has available. Many stock or have ready access to the cloth material that is glued to the backing board. If your headliner backing board is damaged, your best bet is to check salvage yards for a quad cab truck like yours or try LMC truck.

If you get a used on keep in mind you have to remove all the seats to remove it. Same to install, and if you just change the material on yours you need to get the extra wide stuff because the standard 54 inch stuff won't cover all of the headliner.

I bought the plastic lined one from LMC. Mine sure doesn't fit very well. I made it work and you can't really tell so much but hindsight being 20/20 I should have just gotten my original re-upholstered.
