The only two sizes should be 12 something and 13 both will work for your truck. I just looked like like they have 11 and 12 1/8 either will work

Get the 12" kit. I recommend OEM Sachs. My second one was 11" but the third I went with the upgrade (the 12" was an upgrade on 3rd gens with the NV3500 and later trickled down to 2nd gens as required replacements).

The 12" kit has more springs versus the 11" and this aids tremendously in felt engagement. First to second gear is much smoother than on the 11" stock size. Happy shopping.

Doesn't hurt to have the flywheel resurfaced, provided you can find someone to do it, in a timely fashion.... otherwise, just scuff the surface (non-directional) with some 80 grit sandpaper......

I don't know where you saw four sizes, there's only 11 or 12". And 13" are not for the 3500s. This is the exact kit I have. Notice the amount of springs versus the popular Luk. I prefer the OEM brand, Sachs.

And flywheel (notice the size still says 11" but it's the same for 12"):

Use the stock flywheel.

Flywheels are all the same. If yours has any cracks or bad burn marks, replace it. After the first two clutches, I replaced mine. Just be careful, they are heavy!
