O4 Cummins lope at idle
I'm not sure if an injector would behave differently when the motor warmed up bc when it's warm u have no lope at all, also if u just in it and it's looking and then u throw it in drive the loping will stop
I had this issue but it was a tuning issue. It was so far back I do not recall what the problem was in the tune.
Get a scan tool that can log all the sensor data, start the logger then fire it up cold & let it idle until the lope goes away, then stop the log. My bet is it stops roughly at 5 minute mark (that was my problem which pointed me to a tune change I installed the day before). Once you have the data log, if you can scan thru it as it warms up it will show you if a sensor is out of whack.
I use HP Tuners, for reference... MPVI4 – HP Tuners allows me to log data on multiple vehicles that are supported but wont work on all. Not sure what other data loggers are out there to do the same thing.

the smarty touch has data logging... a quick check got me this... SMARTY Touch Data Logging
have a look, give it a shot and lmk what it gets you.

heres a few of them not sure what sensors u think i should moniter, at the end it started idling fine but until then especially after i tapped the brake to get it out of high idle it would lope pretty good
I did install a fass and it seems to be working fine it's been loping for about a year I installed a new fca and new prv as well with all Cummins stuff or Bosch, so should be good quality, but the lope persists untuned, tuned it does it all, it's just weird because it does it speraticly it'll lope 1 second then quit then lope for a whole minute then quit just never the same
They are not cheap so i would look loosely, part of me wants to say a cam/crank position sensor throwing a bad signal, would be fun to probe one with an oscilloscope. Maybe pull the sensor and see if there is any metal particles on them
