Push the brake and the gas in, but only run it a thousand rpms above normal, then punch it as soon as you want to go releasing your brake.

Foot breaking is best. Leave your vehicle in D, Press firmly with your left foot on the brake to hold the vehicle in place, and use your right foot to bring the RPM's up. This loads teh transmission and torque converter up so there is no instant jolt of power to anything.
It should be a smooth transition of power with the weight of the vehicle and your self being pushed back. Hence your forward momentum!!!
Not to sure where to put the RPM's at though. I usually have them up around 2000 - 2500 for a decent launch. But that is me.
Have fun!

for the hemi. . .2-2500rpm and just hold her down with the brake. Cummins, auto 1600-1800rpm and build about 10lbs of boost, stick, 3rd gear2000rpm and use clutch to built ~10lbs. when light goes dump brake and clutch at same time and mat throttle.
if 4x4 take off in 4x4, then aprox half way down track shift into 2wd. this will give you much better power to ground and keep the wheels from spinning too much off the line, after the 1/2 point shift into 2wd, chances of wheel slippage deminish after 3rd gear.
also, for the auto's, use the tow haul, it will lock the TC sooner

I really wouldnt take off with 4wd unless I had the power that would require me to do so. With my Ram 4x4, I launch in D, and keep my foot to the floor.....With Stock horse power, you shouldn't break loose unless there is water on the track or something.....or just have bad tires.
