I drive a 1991.5 D350 CTD with the Getrag 5-speed and average 16mpg around town and 19.5mpg on the highway keeping 70mph with about 6-8lbs of boost cruising. Of course my truck does have some performance mods so I'm happy with what I get!

I've recently straight piped it, and removed the emissions pump, and also bought a tonneau cover for it, I've calculated over 5 full tank fill ups, I'm now at 20mpg.
I've gained around 6mpgs from straight piping, removal of emissions pump, and tonneau cover. WELL WORTH IT.
