It varies too much across the country. I expect you can find someone cheaper there than I can in MA. It also depends on exactly what it needs. It's not usually the parts that are that bad, it's all the labor.
If you want, you could get one out of a boneyard and put it in. Some places will guarantee the parts. Keep your old one so if you decide to rebuild it later you an.

It's not likely a noisy 5th would be due to the throwout bearing. It wouldn't really matter the gear you were in.
Usually a noisy throwout bearing is most obvious when you press the clutch pedal. In my Dak I had one that sounded like a squealing pig no matter what the pedal position. I could play a tune on the thing until it grenaded, literally, and all the ball bearings fell out.
You may have some bearings that need replacing too. Give the boneyards a check, it's worth a shot.

Start by getting some prices from a few shops what they offer you. Don’t forget the new clutch in the price.
Just tell them what is wrong with it have them drive it and explain the problem to them.

call any local 4x4 or Dakota clubs ask for any recommendations regarding your truck and a good trans shop to go to for your AX 15, Jeep clubs are good they use the AX 15 in older Jeeps also.
