Leakdown test won't tell you if valve guides are bad. That requires a physical inspection.

Edit: Of course, the bouncing needle also could be an indication of misfires..... how fast it is bouncing is the way you tell between them.

Damn, and I just had valve covers off. The needle is a blur when it vibrates. The misfire/engine shake seems to not be as bad at times as other times. More-so when engine is warm and been driven a while.

That could be any number of things. I might consider doing a power balance test, with the vacuum gauge hooked up, and see what happens. (disable IAC for the test as well.)

Im rusty on the different things the vacuum gauge can do at idle. I would have to read up with my manuals. I would take HeyYous word on it though he's very knowledgeable

Most of what I've read about the vibrating needle does go along with what you guys are saying. I've found a few places where it states this can be an ignition issue as well. Will hopefully get it figured out soon.

Yep, the engine shake isn't felt bad in the truck, but obvious with the hood up and your hand on the engine. The misfires are random and some are stronger than others.
