How many bolts are there to take the bed off? I can only find four.

There are six bolts.... Two toward the front, two in the middle, and two in the back. Just look for the hockey pucks sitting between the frame, and the bed, that's where you will find the bolts.

Bolts pass thru the center of the hockey pucks. 
They probably hid one above the gas tank, just for you. (not sure, but, that would be just like an auto manufacturer..... I swear the design engineers HATE mechanics.)

I don't think there are any bolts over the tank. From on top of the bed you can see where the front two bolts go. For the tailghts I just removed the housings and fished the bulbs and harness out the bottom.
