Just maybe its fixed. What photos there were of the crank and bearings looked ok. I've had many techs doing that kind of repair plastic gauge and check cylinder for roundness. Just maybe the slight difference of another8 piston took care of it. Usually a tech looks really hard at a engine as they also hope t9hey get to put short/long block in and be done with it. Drive it a month,listen and see if any oil use etc.
Hopefully it's nothing, however I was under ther impression this was farmed out to a machine shop.
As TNTech pointed out from his resources, the journals only have a total of 200 millionth total roundness tolerance. I'm more concerned of the journals then anything else.
The pistons I machine require shoes which ride a thrust bearing plate which ossilate vertically while utilize a wobble plate machined at print spec angles. If the hub on the wobble plate (Which would represent the journal in this particular situation) provided any chatter, scoring, surface finish being tarnished as seen, I wouldn't hesitate to refinish the hub, assuming there is enough tolerance within the overall diameter remaining. This is typically one of the more common scenarios I come across with service operations within or shop.
It just doesn't make sence to replace bearings without refinishing the journals. Even if it required undersized fabricated bearings to properly match the journals, it would have been a more practical route IMHO.

I wasn't clear as to who he was conversing with, at one point he mentioned "machine shop" recording specs.
I'm not sure as to when the whole time frame took place, so I presumed the engine had been pulled, noting he was able to take pics plus the truck being in and out of service for 39 days or so. I'm not trying to beat a dead horse here, just putting my .02 cents in as to what I believe is a correct procedure, since he asked what I thought.

The block wasn't ever pulled. They took 1 head off, 1 piston out, and replaced #1, and #2 bearings. As far as I know, they didn't do anything to the journals.
When I mentioned Machine Shop, it was just that they came out, and properly measured the piston size.
I was able to take pics because the truck was up on the hoist, and I took them from underneath.

Well if it were me I would want a new engine. Now that being said a new engine from the factory is not new. It is a factory rebuilt engine. GM had a name for them called a target motor. These come with a warranty of one year unless there is still a factory warranty on the vehicle then it follows that time cycle or mileage. New motors are for the production line only. That is why there is a core charge on the warranty that the dealer files with the manufacture. If you were buying this yourself that is what you would get. I had a 95 Chevy P/U that had the startup knock. They tried changing filters with a check valve in them. This was a common problem back then. The dealer short blocked the engine and all was well for a while. After the new car warranty was up and the engine did it again I traded it for my first Dodge. I had similar problems with a transmission in a GMC I once owned. They put a factory rebuilt tranny in it to solve the problem. Again no completely new components. If you read the warranty it allows them to do this as long as they guarantee it.
Target engines were new. Sold many of them. Target transmissions were new and reman. Sold lots of them in my day. Usually great price and warranties that competed or surpassed others. . They came with a 36 month 50,000 mile warranty since 1990 0r so. Gm has a AC/DELCO line that was the reman 1 year warranty product. On a in warranty vehicle of this age and mileage it would only be new. Bet Mopar has no reman Hemis anyway. Now there was a time,might still be that with Mopar a lo]t of 'reman stuff wasn't remans. It was new. There is also a tax? reason that some things branded 'reman' and are new.

Still knocking, but quieter then before.
However, since the repair, now the truck doesn't like going into 4WD without it grinding. Here's a video:
